About the Role
The Clinical Patient Engagement Specialist will work directly with healthcare providers to implement and optimize a care coordination platform focused on advance care planning. This role ensures clinical teams effectively use the system to capture patient preferences and improve care alignment.
Responsibilities
- Collaborate with clinical staff to integrate a digital advance care planning platform into daily workflows
- Train physicians, nurses, and care coordinators on system functionality and best practices
- Support data entry accuracy and timely documentation of patient care preferences
- Respond to user inquiries and troubleshoot system-related issues
- Gather feedback from care teams to inform product improvements
- Conduct onboarding sessions for new client facilities
- Monitor adoption metrics and report on engagement trends
- Ensure compliance with clinical documentation standards
- Facilitate workflow optimization using platform analytics
- Serve as a clinical liaison between customers and internal technical teams
- Deliver presentations to interdisciplinary care teams
- Maintain up-to-date knowledge of regulatory requirements related to advance directives
- Assist in developing training materials tailored to different clinical roles
- Participate in client success planning and quarterly business reviews
- Promote consistent use of the platform across hospitals and post-acute facilities
- Track and resolve system usage barriers reported by clinical users
- Support go-live activities for new implementations
- Coordinate with project managers to meet deployment timelines
- Educate staff on how the platform supports care continuity
- Identify opportunities to improve patient-clinician communication through technology
Nice to Have
- Certification in palliative care or care coordination
- Experience implementing health technology solutions
- Background in critical care, geriatrics, or hospice nursing
