Responsibilities
- Oversee patient caseload with a focus on resolving gaps in both clinical and non-clinical care needs.
- Connect patients with community resources that support and enhance their treatment plans.
- Educate patients on managing chronic conditions and improve their understanding of health-related information.
- Conduct medication oversight, including identifying risks, reconciling prescriptions, supporting adherence, and arranging refills.
- Support timely access to essential services such as Home Health, Durable Medical Equipment (DME), and Home Infusion.
- Address patient inquiries promptly and foster open communication to assess individual needs.
- Facilitate referral processing and coordinate appointment scheduling for patients.
