Responsibilities
- Carry out monthly Chronic Care Management calls and finalize verbal enrollments
- Conduct Health Risk Assessments and assist with Transitional Care Management services
- Support Remote Patient Monitoring follow-up and track chronic conditions
- Inform patients and their families about diagnoses, medications, and prescribed treatments
- Work with healthcare providers and internal teams to deliver coordinated, patient-focused care
- Encourage care plan adherence and support patient self-management practices
- Record all patient communications accurately in electronic health record systems
- Follow HIPAA, CMS, and CCS regulatory and quality standards
- Achieve expected productivity and call volume targets while providing compassionate care
